The Locator is for the personal convenience of individual members of the general public to facilitate quick access to information about healthcare providers with whom they may be consulting regarding medical services. Neither the Locator nor any of its data, listings, or other constituent elements may be downloaded, republished, resold, duplicated, used, or reused, in whole or in part, for any purpose other than the personal uses permitted above.
You cannot use the Locator to verify credentials of any particular healthcare provider. The database of healthcare provider information that is used to compile and publish the Locator does not contain sufficient information with which to verify healthcare provider credentials. Inclusion in this database is based on a healthcare provider’s voluntary response to communications to primary care physicians, endocrinologists, urologists and infectious disease specialists. This database is compiled for informational purposes only.
